Risk of death from chronic progressive lung disease depends on type of hospital
October 24, 2003The risk of dying from a sudden worsening of the chronic progressive lung disease COPD depends on the type of hospital at which a patient is treated, reveals research in Thorax. Admissions to hospital for COPD make up around 6% of the total.
Information on length of stay and the number of deaths from COPD up to 90 days after admission was collected over eight weeks from 30 hospitals. In all, information was supplied on 1274 cases.
Death rates in general were high, with 14% of the total number of patients dying during that period. But there was a wide variation in death rates across the spread of hospitals, ranging from 9% to 19% of the total number of cases.
Small district general hospitals fared the worst, with a rate of 17.5%. Teaching hospitals had a rate of 11.9%, while large district general hospitals had a rate of 11.2%. When factors likely to influence the results were taken into consideration, the risk of dying from COPD after admission was still highest at small district general hospitals.
When data on available resources and organisational factors were analysed, the findings suggested that higher death rates were associated with fewer doctors and fewer patients being under the care of a specialist. Small district general hospitals had the fewest resources, the survey revealed.
