During childbirth - a friend in need is a friend indeed.

Women who have a supporter with them throughout labour have much more positive experiences of childbirth. They are also less likely to need painkillers or have any form of surgical intervention. So says a new Cochrane review of fifteen trials that collected data from nearly 13,000 women.

This benefit is greatest if the supporter is not a member of hospital staff, and when the women are in situations where epidural painkillers are not routinely available, such as home-deliveries. Published in The Cochrane Library, the research shows that using nurses or midwives to provide this support may not be as effective because they are often highly influenced by the technology-driven culture of the delivery suite.

Since the middle of the 20th century there has been a significant move away from the age-old practice of women supporting other women during labour and birth as the birthing process has moved from home to hospital. There are now concerns that this has dehumanised childbirth.

Having an experienced woman with you during labour has many advantages. She can give emotional support, encouragement and praise, as well as advice about coping techniques. In addition she can help articulate a woman's wishes to others.

"Every effort should be made to ensure that a woman's birth environment is empowering, non-stressful, affords privacy, communicates respect, and is not characterised by routine interventions that add risk without clear benefit," says lead author Professor Ellen Hodnett, RN, PhD. She concludes "Continuous support during labour should be the norm, rather than the exception."
