Acute Heart Failure is a complex acute care area since the reasons for Acute Heart Failure can be related to coronary heart disease and its complications in 50-60%, arrythmias in 10-30% of the cases to valvular heart disease or cardiomyopathies in 5-10% of the cases. In addition, there can be causes related to myocarditis, septicemia & post-operative complications, but these are not very frequent. The advised treatment of Acute Heart Failure is based on a large number of small trials as larger controlled and evidence based trials are rare. This is mainly due to progress in treatment, which has only lately seen new agents and otherwise based on medications since 1970s. The Acute Heart Failure Guidelines clarify the nomenclature for Acute Heart Failure; they describe the pathophysiology of Acute Heart Failure, the non-invasive and invasive monitoring of patients and the nonpharmacological and pharmacological treatment of patients. The Task Force has worked to classify the recommendations and evidences on all these aspects. The Task Force has also outlined recommendations for organisations and how therapies should be verified in documentation of new pharmaceutical agents. This has been an ongoing debate with pharmaceutical agencies. The rules have been constantly changing and evolving. These Task Force guidelines emphasise the evaluation of recently introduced drugs, the relief of symptoms and clinical findings including diuresis and biochemical findings, quality of life and effects on outcome and prognosis. Since ischemic heart disease is a major reason and a complicating factor in Acute Heart Failure, there are several chapters on the care of acute coronary syndromes and complications of acute myocardial infarction. The Guidelines also define the role of devices and assist devices. The recommendations have an extensive summary on initial, secondary and tertiary care of Acute Heart Failure.
