DEATH RATE AMONG UK GULF WAR VETERANS (p17)
June 28, 2000Armed forces personnel who served in the Gulf War report more current ill health than those who were not deployed. There has been concern expressed that they may also experience higher mortality rates. In this week's issue of The Lancet Gary J Macfarlane and colleagues report the findings of a retrospective cohort study that included all 53 462 UK Gulf War veterans (Gulf cohort) and a comparison group matched for age, sex, rank, service, and fitness level, but who were not deployed in the Gulf (Era cohort).There were 395 deaths among the Gulf cohort and 378 deaths in the Era cohort. Death from "external" causes was higher in the Gulf cohort (Gulf 254, Era 216), whereas death from "disease-related" causes was lower (Gulf 122, Era 141). The higher mortality rate from "external" causes in the Gulf cohort was mainly due to higher mortality rates from accidents. There was no excess of deaths recorded as suicide in the Gulf cohort.The investigators conclude: "This follow-up of the Gulf War veterans has shown, 8 years after the end of the conflict, that although they have experienced higher mortality rates than a comparison cohort, the excess mortality rate is very small and does not approach statistical significance."The researchers discuss why there should have been more deaths from motor vehicle accidents and mention one possible hypothesis, namely that those who have been on active service may subsequently have different relative perceptions of risk and may participate in high-risk activities and behaviours. The investigative team stresses that it is too early to detect any possible increases in disease-related deaths from, for example, cancer.
