Screening mammography in elderly patients beneficial

April 14, 2008

Although guidelines keep changing regarding screening mammography in elderly patients, those older than 70 years old continue to benefit from this exam, showing that with frequent mammograms breast cancers can be found sooner, according to a recent study conducted by researchers at Jacobi Medical Center and Albert Einstein College of Medicine, in Bronx, NY.

"Our initial interest was sparked by the changing mammographic guidelines in the elderly age group and the differences in the guidelines between organizations including the American Cancer Society and American College of Radiology," said Jason Salsamendi, MD, lead author of the study. "We also noticed that we were performing a significant number of screening mammograms in elderly patients at our institution and became curious about the incidence of occult breast cancer in this age group," he said.

The study consisted of the review of 24 cases of breast cancer in 22 elderly patients age 70-89.

The researchers evaluated the mammograms for masses and calcifications and correlated the results to surgically proven histology, lymph node involvement and initial surgical management.

According to the study, 19 of the 24 cases of breast malignancy were initially identified on screening mammography. The study showed that mammographic findings in these 19 cases included 12 masses, 4 suspicious calcifications and 3 masses with calcifications. Of these, surgical management was available in 18 cases with 15 being treated by lumpectomy and 3 being treated with mastectomy. The study also showed that the average time interval from most recent prior mammogram to diagnosis was 2.6 years.

"During our study, we found that five patients never had a prior mammogram. Their breast cancer was detected on a baseline study. These five patients included three cases of infiltrating ductal carcinoma," said Dr. Salsamendi. "Perhaps with more frequent mammographic screening, more breast cancers can be found earlier, at a noninvasive stage. This would result in a less extensive surgical procedure and improve prognosis. Our results encourage clinicians to continue to order screening mammography in elderly patients at yearly intervals," he said.

Obese women play cancer roulette
Obese women may be putting themselves at greater risk of breast cancer by not undergoing regular screening. According to new research by Dr. Nisa Maruthur and her team from The John Hopkins University School of Medicine in Baltimore, USA, seriously obese women are significantly less likely to say they have undergone a recent mammography than normal weight women, especially if they are white.

The simple application of a pain-relieving gel may reduce the breast discomfort some women experience during mammography exams, according to the results of a clinical trial published in the online edition of Radiology.

New mammography technology improves cancer detection
A new radiological diagnostic tool called stereo mammography allows clinicians to detect more lesions and could significantly reduce the number of women who are recalled for additional tests following routine screening mammography.

MammoTrainer: Interactive Training for Breast Cancer Screening Mammography

MammoTrainer: Interactive Training for Breast Cancer Screening Mammography
by J.H.C.L. Hendriks (Author), Roland Holland (Author), Henny Rijken (Author)

The MammoTrainer is a self-training software for PC that allows you to improve and test your skills in soft-copy reading of mammograms. It is built on a database of 250 real screening cases (200 normal cases, 50 histologically proven cancers) with 2,000 mammograms from the Dutch national breast cancer screening program. Specify a set of training cases, read them with typical soft-copy tools (e.g. contrast/brightness, zoom, CAD), localize and classify lesions, and compare your findings with the confirmed pathological diagnosis. Based on a wealth of mammographic cases, each with current and prior images, the MammoTrainer is a useful assistant for training and preparation both in diagnostic and screening mammography.

Program I (30 min.) -- Genitourinary injury is often very subtle and sometimes not apparent until several hours after it occurs. This program reviews the diagnosis, evaluation, and management of both upper and lower genitourinary trauma in the acute care setting. Program II (12 min.) -- The questions of which women to screen for breast cancer by mammography, at what age, and how often have generated more controversy than perhaps any other medical intervention. This program reviews the data on screening women under age 50 and attempts to provide an evidence-based foundation for informed decision-making by health care professionals.
