Preoperative radiation may improve survival rates in advanced rectal cancer patients
December 02, 2008
Geisinger expert: Care should be individually tailored WILKES-BARRE, PA. - Patients treated with radiation prior to surgery for advanced rectal cancer have fewer instances of cancer recurrence and better overall survival rates, according to a recent Geisinger report. The report examined a treatment called neoadjuvant therapy, which can reduce cancerous tumor size or limit the spread of cancer, before surgery to remove the tumor. Neoadjuvant therapy may include chemotherapy and/or radiation. Neoadjuvant therapy should not be considered a "one size fits all" approach for rectal cancer patients, said the report's primary author, Mohammed Mohiuddin, MD, FRCR, FACR. Dr Mohiuddin is director of The Henry Cancer Center and co-director of Geisinger Health System's Cancer Institute. "Physicians need to consider a variety of factors such as tumor size, cancer stage and patient preference before deciding on the course of preoperative treatment," said Dr. Mohiuddin, who has published extensively on the topic of radiation oncology. The report compared the results of nine recently published research studies involving several thousand cancer patients in Europe and the United States. Giving comparatively higher dosages of radiation reduces the likelihood that the cancer returns to the same place in the body, the report said. Short-term radiation therapy in some patients may be more convenient because it reduces the number of trips to the hospital. In patients with late stage rectal cancer, the report supports the pre-surgical practice of combining chemotherapy with smaller radiation dosages over several weeks. Geisinger Health System

MRI of Rectal Cancer: Clinical Atlas
by Arnd-Oliver Schäfer (Author), Mathias Langer (Author)
Today magnetic resonance imaging (MRI) of rectal cancer is one of the most interesting topics in radiology. Over the last decade technical refinements have strengthened the role of MRI as a first-line diagnostic strategy for accurate staging of rectal cancer. MRI is perfectly suited to predict tumor-free circumferential resection margins. Additionally, high accuracies have been reported for the T and N stages, which is comparable to transrectal ultrasound (TRUS). Novel approaches to moving table MRI serve as a platform for thoracic, abdominal and pelvic staging of rectal cancer patients within one examination. The broad spectrum of high-quality illustrations and the contribution of clinical experts provide substantial insight into the sophisticated field of rectal cancer management.

Modern Management of Cancer of the Rectum
by Riccardo A. Audisio (Editor), James G. Geraghty (Editor), Walter E. Longo (Editor)
Modern Management of Cancer of the Rectum is intended to provide a comprehensive overview of all aspects of rectal neoplasms. It addresses such topics as epidemiology, biology, screening and chemoprevention, the role of imaging in diagnosis, staging and prognosis, radiation therapy, medical and surgical treatment, as well as new modalities of therapy, including laparoscopy, and transanal endoscopic surgery. A greater understanding of prognostic factors, patterns of spread and natural history has occurred during the past decade; together with new diagnostic modalities this has led to significant changes in the management of patients with rectal cancer. This book will be invaluable for all those who treat rectal cancer.

Cancer Recovery Guide: 15 Alternative and Complementary Strategies for Restoring Health
by Jonathan Chamberlain (Author)
In Europe and the USA we have a 40-50 percent chance of getting cancer at some time in our lives. So what do you do if you are diagnosed with the disease? The harshness of orthodox treatments (surgery, radiation and chemotherapy) are well-known. Their use is widespread, but their results are not impressive. Faced with these options, informed patients are increasingly seeking out alternative or complementary strategies to take control of their own healing. This book provides an overview of them. Jonathan Chamberlain watched his wife suffer and eventually die - both from her cancer and from the direct effects of the orthodox treatments she had undergone. This experience led him on a journey in search of other methods of overcoming cancer. What he discovered stunned him. There are cures out...

Transanal Endoscopic Microsurgery: Principles and Techniques
by Peter Cataldo (Editor), Gerhard F. Buess (Editor), D.J. jr. Schoetz (Editor)
Transanal endoscopic microsurgical techniques have recently been proven a safe and effective way to remove tumors from the rectum. Patients with small (4 cm in diameter or less), early malignant tumors are candidates for TEM as are patients with benign tumors of any kind. Preoperative biopsies and staging are essential, and patient selection is key to achieving successful outcomes with these procedures. Transanal Endoscopic Microsurgery covers indications/contraindications, equipment, operative techniques, oncologic results, and an evidence-based comparison of TEM and traditional approaches. Written by the innovators of these techniques, this book is an essential reference for surgeons who want to incorporate TEM into their practice.
