Driven by advances in digital imaging, energy-based devices and robotic platforms, minimally invasive laryngeal surgery has shifted from simple lesion resection toward precision functional preservation. High-definition endoscopy with narrow-band imaging improves early lesion detection as an “optical biopsy,” while updated laser systems, low-temperature coblation and transoral robotic surgery enable more precise tissue manipulation with less thermal damage and faster postoperative recovery. Flexible and single-port robotic systems have further expanded surgical access to hard-to-reach subglottic and hypopharyngeal regions.

Alongside device upgrades, surgical strategies now prioritize vocal function reconstruction, with optimized microflap techniques and autologous tissue grafts improving outcomes for challenging conditions such as vocal fold scar and sulcus vocalis. The review also notes expanded use in laryngotracheal stenosis and congenital neck lesions, plus promising progress in tissue-engineered injectable materials and AI-assisted intraoperative navigation. The authors acknowledge remaining challenges including limited haptic feedback in robotic systems, high equipment costs and steep learning curves, and call for more integrated, cost-effective solutions to broaden clinical access.
