The research article published on May 7, 2008 in the World Journal of Gastroenterology, demonstrates that caudate lobe is the first segment of liver in Couinaud's classification. Because of its unique anatomical site, its resection has a high risk. It can be safely resected if an appropriate operation procedure is used.
Clinical data obtained from the caudate lobe of 11 primary liver cancer patients who received hepatectomy were retrospectively analyzed. Four safe procedures were used in operation: an appropriate skin incision for excellent exposure of operative field, adequate mobilization of liver to displace it upwards to the left or right, placement of tapes for total hepatic vascular exclusion to control fatal blooding when necessary, and an ideal procedure for hepatectomy and removal of multiple lobes.
This paper is interesting as it describes a series of caudate lobe resection, which indicates that caudate lobectomy for cancer in candate lobe can be safely performed when an appropriate procedure is used.
