Artificial intelligence (AI) is rapidly transforming pediatric surgical care, offering tools to enhance diagnostic accuracy, refine surgical planning, and personalize postoperative management. From predictive risk calculators to AI-assisted intraoperative diagnostics and automated documentation systems, these technologies promise to improve efficiency while supporting family-centered decision-making. Yet when applied to children—whose voices are represented by parents and guardians—the ethical stakes are uniquely high. This perspective explores how core principles of medical ethics must guide the responsible integration of AI in pediatric surgery, emphasizing human dignity, transparency, accountability, fairness, and the preservation of trust in an era where algorithms increasingly influence critical clinical decisions.
Technological innovation has always driven surgical progress, and AI now represents the next transformative wave. Machine learning models are now being developed to predict surgical risks, assist in diagnosing rare congenital disorders, analyze imaging data, and anticipate postoperative complications. Risk prediction tools have already shifted from traditional statistical methods to more complex machine learning approaches, improving their ability to account for nonlinear interactions. However, pediatric populations present unique challenges: small sample size, developmental variability, and underrepresentation in large datasets, increasing the risk of bias and inaccurate predictions. Concerns about privacy, cybersecurity, and the opaque "black box" nature of deep learning systems further complicate clinical adoption. Based on these challenges, in-depth research is urgently needed to establish robust ethical and governance frameworks for pediatric surgical AI.
